THE PROFILE OF HEPATOMA PATIENTS HOSPITALIZED AT INTERNAL MEDICINE RSMH PALEMBANG

Background. Hepatoma is a malignant cancer of liver that attacks hepatocytes, the prevalence of hepatoma covers 5.65% of all cases of cancer in humans and reaches the fifth rank of men and the ninth rank of woman as the most common cancer in the world. Knowledge of the profile of hepatoma patients in RSMH is still quite limited.
The Aim of The Study. The aim of this study is to know the profile of hepatoma patients hospitalized at Internal Medicine RSMH Palembang Januari-December 2009
The method of the study. This study is a retrospective descriptive study to reevaluate the medical records of hepatoma patients hospitalized at Internal Medicine RSMH Palembang, Januari-December 2009
The Result of the Study. This study discovered 50 cases of the diagnosis of hepatoma. From those cases, the most common age suffering from hepatoma is 41-60 years old (48 %), the most common patient was male (74 %), and most of them lived at Palembang (54 %). It was found that the most risky factors were hepatitis B(24%), the most common complains of the patients were the pain in right upper quadrant(KKA pain) (76%), and the most common physical examination was hepatomegaly (86%). The laboratory results of these patients had increased such as alkaline phosphatase (48%), SGOT(43%), SGPT(66%), total bilirubin (64%), direct bilirubin (72%), indirect bilirubin (48%), gamma-glutamyl transferase (GGT)(50 %), AFP (18%), while for positive HBsAg (26%), HBsAg negative (34%),Anti-HCV Positive (2%), and Anti-HCV negative (24%). USG examination that was found most frequently to “ekostruktur lesi” were hiperekoik (38%), type of lesion was multiple nodules (46%), the infected lobe was 1 lobe (22%). The most common complication was metastasis (20%)
Suggestion. In this case, counseling to the society about hepatoma and factors causing hepatoma is really necessary so that they can prevent this disease before it's too late. Besides that, it’s important to do USG screening and AFP examination every 6 month of patients with hepatitis B and C, as well as patients with cirrhosis of liver to detect the hepatoma in the early stages in order to have a better prognosis. The last is the need of conducting further research with larger sample and more meaningful variable in the diagnosis of hepatoma and and determination of the level of aggressiveness or severity of hepatoma.
